PROSTOX Ultra and PROSTOX Standard are new and innovative tests, and as such, coverage may vary depending on your insurance provider. If your insurance does not cover PROSTOX Ultra or PROSTOX Standard, you may still be eligible for significant cost savings through the MiraDx Financial Assistance Program, which offers reduced pricing based on household income. Eligible patients may pay as little as $0, with a maximum out-of-pocket cost of $395 per test.
MiraDx Billing Process
Medicare Patients
At this time, Medicare does not have a specific coverage policy for PROSTOX Ultra or PROSTOX Standard. Please note that you may receive a Medicare Summary Notice (MSN) or other communication from Medicare indicating that the test was not covered, however, these notices are informational and do not mean that you owe any payment to MiraDx for testing.

No Surprises Act
Good faith estimates: If you are uninsured or choose not to use insurance, you have the right to receive a Good Faith Estimate from MiraDx. This estimate outlines the expected cost of your lab testing before services are provided.
- It includes a breakdown of the anticipated charges for your test(s).
- You can request an estimate at any time before testing.
- If your final bill is $400 or more above the estimate we previously provided to you, you may be eligible to dispute the charge.
