PROSTOX Ultra and PROSTOX Standard are innovative diagnostic tests, and insurance coverage may vary depending on your health plan and provider. At MiraDx, we are committed to helping patients understand their options and navigate the financial aspects of testing.
If your insurance does not cover PROSTOX Ultra or PROSTOX Standard, or if you have out-of-pocket costs after insurance processing, MiraDx offers a range of support options, including payment plans and a Financial Assistance Program for eligible patients.

The MiraDx Financial Assistance Program is designed to help reduce financial barriers to testing by providing reduced patient responsibility based on household size, household income, and other program criteria. Eligible patients may qualify for significantly reduced costs and, in some cases, may have no out-of-pocket responsibility. For eligible patients, out-of-pocket costs will not exceed $295 per test.

Medicare Patients
At this time, Medicare does not have a specific coverage policy for PROSTOX Ultra or PROSTOX Standard. Please note that you may receive a Medicare Summary Notice (MSN) or other communication from Medicare indicating that the test was not covered, however, these notices are informational and do not mean that you owe any payment to MiraDx for testing.

No Surprises Act
Good faith estimates: If you are uninsured or choose not to use insurance, you have the right to receive a Good Faith Estimate from MiraDx. This estimate outlines the expected cost of your lab testing before services are provided.
- It includes a breakdown of the anticipated charges for your test(s).
- You can request an estimate at any time before testing.
- If your final bill is $400 or more above the estimate we previously provided to you, you may be eligible to dispute the charge.
